Reactivation of Dormant Certificate

Application Instruction Sheet

1. Please fill out all lines on the Application for Reactivation of Dormant Certificate. Please use black ink.
2. Read and acknowledge that you understand and agree to abide by the ABC Rules of Professional Conduct by signing Form ABC 500.
3. Have your immediate supervisor or laboratory director verify and sign for all information provided on your application. If you are a consultant without an immediate supervisor or laboratory director, you have two choices. You may have an ABC Certificate Holder who is familiar with your work sign the form. Or, you may have three (3) different clients fill out ABC Form 701 verifying that you completed work for them in the past year.
4. There is a $50 non-refundable fee for the Reactivation of your Certificate. Make your check or money order payable to the American Board of Criminalistics, Inc.
5. Please include the original plus 2 copies of the Application for Reactivation of Dormant Certificate, the Rules of Professional Conduct and your $50 check to:
